(Available to those who have registered their Individual Number with the Health Insurance Society)
1. Expiration-date extension stickers to be attached to insurance cards
(Applicable to those enrolled in health insurance as of August 31, 2024)
Your current insurance card expires on November 30, 2024.
The purpose of this sticker is to extend this expiration date to December 1, 2025 (the transitional measure deadline).
How to attach the expiration-date extension sticker to your health insurance card

<Note>
- ◆Note that only your current Bosch Health Insurance Society insurance card can have its expiration date extended by attaching a sticker in this way. Do not under any circumstances attach or reuse the sticker for other health insurance cards.
- ◆When you attach the sticker, only cover the expiration date area, and be careful not to hide any other indicated information.
- *In particular, the expiration date area of a dependent’s insurance card is extremely small, so be especially careful when attaching the sticker.
2. Your Insurance Eligibility Notice
(Available to those who have registered their Individual Number with the Health Insurance Society)
Bosch Health Insurance Society notifies people of their eligibility information while also notifying them of the last four digits of their Individual Number to verify that registered Individual Numbers are correct.

<Note>
- *You can use the credentials displayed by accessing Mynaportal in place of the "Your Insurance Eligibility Notice".
