* Handling of application forms, user charges, etc., varies from center to center, so please contact to the Management association or recreation facility reception.

Once you make reservation
- STEP2Send an Application Form to the Health Insurance Society.
<by internal mail, standard mail, or in person>
* The Application Form is on the Health Insurance Society Association website - STEP3You will be sent the Application Form bearing the Health Insurance Society stamp by the Health Insurance Society.
- STEP4Mail the Application Form to the application address.
* Please include a self-addressed, stamped envelope! - STEP5You will be sent an Notification Form
- STEP6Present your Notification Form when you visit the center, and pay any user charges at the center.
